  • Pelvic Floor Physiotherapy refers to the treatment of Pelvic Floor Dysfunction (PFD) which is common across all ages and affects both men and women.

Introduction

Our pelvic floor is important in our recovery from childbirth, gynecological surgeries or prostate surgeries or treatment. Our pelvic floor changes as we age.

If you have suddenly found yourself running to the bathroom more frequently or ever been afraid to laugh because you may lose bladder control, unintentionally pass gas, strain to have a bowel movement or experience heaviness in the pelvic area at the end of a long day you may suffer from PFD. The number of children you have had can also affect your pelvic floor.

Men also experience difficulties with bladder control as they age and particularly after prostate treatments. Children and young athletes also can also experience difficulties with bladder control. One in three women suffer from pelvic floor dysfunction.

Pelvic Floor Dysfunction

  • Your pelvic floor muscles are like a sling that supports the group of muscles in your pelvic area. It goes from your pubic bone to your tailbone and supports your bladder, bowel, and the uterus in women. It includes muscles, ligaments, connective tissue, and nerves
  • Your pelvic floor muscles allows you to control your bowel and bladder movements and play a role in sexual function
  • Pregnant women can benefit from pelvic floor training to help their with pregnancy related pain and help with bladder and bowel function before and after delivery of baby
  • PFD can be due to muscles that are too weak but can also be due to muscles being too tight.
  • For optimal function, you want a pelvic floor that can adapt so it can tighten and relax when needed

For a number of reasons, the pelvic floor can become dysfunctional when these muscles are not working well together, this can lead to urinary incontinence (involuntary loss of urine) and your pelvic organ dropping (prolapse). There are two main types of urinary incontinence:

  1. Stress urinary incontinence (SUI) is the involuntary loss of urine because of coughing, sneezing, or physical activities such as sports. This is the most common type of urinary incontinence. Urge urinary incontinence (UUI) is the involuntary loss of urine associated with the urge to urinate.
  2. Urge incontinence can result in leaking or frequent trips to the bathroom or it can cause a constant feeling of always having to urinate. Urge incontinence is often accompanied by increased daytime frequency of going to the bathroom and/or at night (this is called nocturia when you get up in the night to go to the bathroom). This is more common in menopausal women.

Men can experience incontinence after prostate treatment or surgery and as they age.

Men and women can suffer from pelvic pain. Pelvic pain can be caused by several factors. If the pelvic floor muscles are too tight or cannot relax then this can be one of the reasons that causes pelvic pain.
